Thrive at Home was founded by three experienced nurses who shared a common observation throughout their careers in acute care. Bringing together over 50 years of combined hospital-based experience, they witnessed firsthand the stress and vulnerability families face when transitioning from hospital to home.
Our founders recognized a critical "gap" in the community: the need for clinical-grade care that doesn't feel clinical. They saw patients returning to environments that couldn't support their complex needs, leading to readmissions and a loss of independence.
